I understand that I will need to pick up my child's birthday bag at the location I chose above.  If I do not show up then I possibly forfeit the bag.  I also understand that I need to be a resident of Bartholomew County to qualify for this program.  I have provided correct and accurate information on the form above and understand that all items in the bag are filled with donated inventory that is every-changing and EGF does it's best to accommodate all choices.  Bag requests must be made the month prior to the child's birthday and for ages 1-18 years.  EGF or pick up locations are not responsible for accidents or incidents.
